Male Sexual Dysfunction

In men, sexual dysfunction refers to difficulties engaging in sexual intercourse. Sexual dysfunction encompasses a variety of disorders that affect sex drive (libido), the ability to achieve or maintain an erection (erectile dysfunction, or impotence), ejaculation, and the ability to achieve orgasm. Sexual dysfunction may result from either physical or psychologic factors. Many sexual problems result from a combination of physical and psychologic factors. A physical problem may lead to psychologic problems (such as anxiety, fear, or stress), which can in turn aggravate the physical problem. Sexual dysfunction may lead to psychological, biological and social ramifications. As such, it is important to remember that, whatever the cause; treatment will require a holistic approach to achieve adequate resolution of the problem.

The main areas of sexual dysfunction in men are :

• Desire - decreased interest in sexual activity, lack of spontaneous sexual thoughts. Possible causes include psychological factors (such as depression, anxiety, or relationship problems), drugs, and low levels of testosterone.
• Erection - no spontaneous or nighttime erections, erectile dysfunction (is the inability to achieve or maintain an erection adequate for penetration). The cause may be a disorder that reduces blood flow or damages nerves to the penis, a hormonal disorder, use of certain drugs, or psychological issues.
• Ejaculation - decreased pleasure from ejaculation, decreased volume of or no ejaculate, premature ejaculation, retrograde ejaculation (where semen passes into the bladder rather than along its usual route down the urethra). Inability to ejaculate (an ejaculation) is usually caused by inability to reach orgasm. It usually occurs as part of erectile dysfunction.

Sexual problems are common throughout the world. In the United States it is estimated that the sexual dysfunction (SD) prevalence among men to be 31%, and an analysis of 22 general population surveys conducted in western countries summarized the estimates of the prevalence of SD for men (inhibited climax in 5%, premature ejaculation in 35%, inhibited sexual excitement in 10% to 20%, and inhibited sexual desire in 1% to 15%).

1. Small Caltrops (Tribulus terrestris):

The plant Tribulus terrestris (TT) popularly known as puncture vine or Small Caltrops is a perennial creeping herb with a worldwide distribution. Tribulus terrestris (TT) has long been used in the traditional Indian and Chinese systems of medicine for the treatment of various ailments such as urinary infections, inflammations, leucorrhoea, oedema and ascites (Chopra et al., 1958; CHEMEXCIL 1992) and is popularly claimed to improve sexual functions in man.

Tribulus terrestris is known for its aphrodisiac activity and it has also been proved in many scientific studies that Tribulus induces erection by increasing intracavernosal pressure and improves the overall sexual function in males. It rejuvenates and nourishes the body.Active principle of Tribulus terrestris is protodioscin, which exerts proerectile aphrodisiac activity by increasing androgen levels and subsequent release of nitric oxide from the nerves of the corpus cavernosum muscle of the penis.

It is observed in scientific studies that Tribulus terrestris exerts a positive effect on the spermatogenesis.In a clinical trial, Tribulus terrestris was given to 74 infertile men for 3 months. A significant improvement was noted in patients receiving the Tribulus supplement. 100% of patients showed an increase in sperm count and motility. Surprisingly 87% of couples conceived within 6 months of treatment.⁷

2. Indian-Spider plant (Chlorophytum arundinaceum):

Chlorophytum arundinaceum is the vital ingredient of number of Ayurvedic aphrodisiac formulations mentioned in the ancient literature. Chlorophytum arundinaceum is an Aphrodisiac, Libido enhancer, and tonic in action. It Increases the sperm count and is also used in spermatorrhoea.

In an experimental study on rats, it has been proved that extract of roots of Chlorophytum arundinaceum has a potent sexual stimulant action.

Roots of Chlorophytum can be useful in the treatment of certain forms of sexual inadequacies such as premature ejaculation and oligospermia . It also exhibits anti-oxidant action.

3. Velvet bean (Mucuna pruriens):

The seeds of Mucuna pruriens are being used for sexual debility, impotence and diabetes. The plant is reported to have L-Dopa as a major constituent mainly in seeds. The traditional aphrodisiac property of Mucuna pruriens is scientifically analyzed and proved by several experiments using seeds of M. pruriens. According to the studies of Amin et al, M. pruriens stimulates sexual function in normal male rats which was observed by increase in mounting frequency, intromission frequency and ejaculation latency.

In a clinical trial on 60 psychologically stressed infertile men, it was proved that Mucuna pruriens increases sperm concentration and motility in all the infertile men.Treatment with M. pruriens regulates spermatogenesis and improves semen quality in infertile men. it possesses anti-oxidant, neuroprotective and anti-stress activities.

4. Salep orchid (Orchis latifolia): :

The Orchis latifolia has been used as a potent aphrodisiac in Ayurveda for the treatment of Male infertility. It has also been used as general tonic . Orchis latifolia nourishes reproductive organs, promotes spermatogenesis, increases testosterone hormone secretion and is helpful for male sexual dysfunction.

It has been observed that hyperglycemia causes adverse/deleterious effect on male sexual function.   Orchis latifolia significantly reduces the deleterious effects of hyperglycemia on male sexual function.

5. Hygrophila (Asteracantha longifolia):

Asteracantha longifolia is Rasayana, general tonic, aphrodisiac and it has androgenic activity. It helps in spermatogenesis. It also possesses strong anti-oxidant and hepatoprotective activities

6. Pellitory (Anacyclus pyrethrum):   

Anacyclus pyrethrum is widely recognized in Ayurvedic system of medicine for its use as aphrodisiac, tonic and a remedy to improve sperm count. Anacyclus pyrethrum is being used as a remedy todelay the ejaculation and to prolong the sexual intercourse.

In a research study Anacyclus pyrethrum has shown androgenic effect. Anacyclus pyrethrum enhances spermatogenesis and improves the quality of seminal parameters. In research study it has been observed that Anacyclus pyrethrum helps to increase penile erection similar to the increase in penile erection index observed by administration of testosterone.

Traditionally, Anacyclus pyrethrum is used as tonic to the nervous system and its antidepressant activity is scientifically established.
